Prorated Cremation Benefits (First 12 Months)

Months 1–6
The cremation benefit begins with $500 off the current price of a
direct cremation service from an approved Dignity4Less Preferred Provider General Price List,
plus a 10% discount off regularly priced merchandise.

Months 7–12
From six months through the one-year anniversary, the cremation benefit increases to
$1,000 off a limited direct cremation service from an approved
Dignity4Less Preferred Provider General Price List,
plus a 10% discount off regularly priced merchandise.

Note: Sales tax, merchandise, transportation, and third-party fees are not included.

• Membership must be purchased before age 75
• Benefits redeemable only through approved Dignity4Less Providers
• Limited direct cremation plan
• Deaths outside Ohio may incur additional fees
• This membership is a discount/benefit program and is not insurance

No Refund Policy

Because benefits, discounts, and access begin immediately upon enrollment,
all membership payments are non-refundable once processed.
